From ProPublica, both nationally and by state, covering machine malfunctions, paper ballot shortage, inadequate facilities, illegal challenges, menacing poll watchers, etc.

Threat Level is doing a pretty good job on this as well; especially the bogosity involved in electronic voting machines.
